We provide professional, affordable home care with physician leadership.

Fountain Community Healthcare Services was founded and is owned and operated by a medical doctor, Stephen Kponyoh, MD. Registered nurses conduct routine assessments and help supervise and train caregivers.

Our Kentucky and Ohio offices share one purpose: to help people receive the care they need in their own homes, where costs are lower and patients are often happier.

Both locations offer long-term support, including 24-hour and live-in care. Available clinical and supportive services differ by state, allowing each office to meet the needs of the communities it serves.

Fountain Community Healthcare Services takes pride in our care professionals. Our homemakers, companions, certified nursing assistants (CNAs) and personal care aides undergo a rigorous screening process, including:

Background and reference checks
Initial and annual skills evaluations
License and/or certification verification

Our quest for the best possible employees does not end with hiring. Caregivers continue to update their skills through training we provide. An RN supervises our caregivers throughout their time with you.

Our nursing supervisors will carefully assess the skills needed in your home and stay attuned to the personality of each client. The nurse will then match the assistant with the right skill set and compatible personality to your account. We want you to feel comfortable with the care you receive.

Should your caregiver ever be out sick or on vacation, our team will work to assign an excellent replacement and notify the client to expect a new face temporarily.

Our non-medical aides are trained to check vital signs and can help clients stay on track with ordered blood work used for chronic disease management and health screenings, under appropriate clinical direction.
